Lay abstract: A 50-year-old man who had suffered from daily obsessive-compulsive disorder (OCD) symptoms for 39 years, in addition to fibromyalgia and irritable bowel syndrome symptoms, was enrolled in a randomized double-blind placebo-controlled clinical trial to test the effects of a low-glutamate diet on fibromyalgia/irritable bowel syndrome symptoms. After 1 month on the low-glutamate diet all of his symptoms remitted, including his OCD, which had previously been nonresponsive to pharmacological treatment. This case study is limited by self-report of symptoms; however, glutamatergic neurotransmission appears to be dysregulated in OCD, suggesting biological plausibility for this observation. Future research is needed.
